There will be an out of pocket cost when receiving care at Leeton Family Clinic. Note that billing is at the doctors discretion, however, we are dedicated to providing your family with the highest level of primary care, and this may mean that a cost applies to your appointment.

Please note that a fee may be charged if patients miss more than 3 appointments

You can contact our admin team and complete a transfer of records form, we can then arrange to fax this through to your previous practice. Usually, records are received within 6 weeks.

You can complete a transfer of records form at your new practice, and once received, we will be in touch with you to process our administration fee of $55. Once processed, we will transfer the records within 6 weeks.

Yes! Telehealth appointments are available for eligible patients. Please contact reception to check your eligibility and book in accordingly. Only patients who have been seen face to face by this practice in the last 12 months are eligible for telehealth services.

Yes, you can process this request via Automed. Note that for some medications and referrals, a consultation will be required. If it is repeat medication, you may be eligible.
